• All unmarried adults must fill out a seperate application
• Married couples can fill out one application

2. Complete, sign, and attach all necessary income documentation

• Please fill out all areas completely (missing information will cause a delay in approval)

3. Fax application & go to PayLease to pay the application fee PayLease

• $25 fee per adult must be paid before application can be processed
• Please fax application to 909-652-9620 (fax)
